He just was not used to her yet.

I don't think a little nip would have killed her...he had to have put his mouth around her body and bit pretty hard to have punctured a lung. It's not that you are being punished for what happened. You just need to think of the best interest for any puppy you're thinking of bringing into your life. You already have one dog - your lab, and unfortunately he is untrustworthy around small breeds. You can say it was an accident and it probably was, but it still happened, and it could happen again.
