They pinch the skin tight with pliers, then insurt a needle and then thread the ring through. There was a lot of blood. They recommend washing it 3 times a day with antibacterial soap. Wash the ring really good with soap that move it back and forth through the hole. After about 2 weeks it will start to use white stuff. These are white blood cells healing the wound. If it's yellow worry. Mine got slightly infected, but i didn't take it out i just kept washing it and used a little peroxide. The infection cleared up pretty quickly. If you take it out before 6 months the chances of getting it back in are pretty slim. I had my done twice. The first time healed up quickly and just left a little scar, but i scar bad so that might just have been me. I refused to take mine out when it got infected because of how much it cost. $60.
