We never sleep with the comforter pulled up and we don't sleep on top of it. Sometimes, Max will lie down on it at the end of the bed for awhile if he's waiting for me to come to bed, but he usually wants to be right next to us if we're in bed. I have extra blankets on a bench at the end of the bed if we get cold.
I've used the ribbon method before and it really helps a LOT. This particular duvet and cover seem to stay together pretty well, so I didn't need to do it with it.
