I think you should have her x-rayed a week before you think she's due to see what size the puppies' heads are. If the vet thinks any appear to be too big to pass then he can schedule a c-section. If it looks like she has a chance of passing them herself, I'd try a natural delivery. Be sure to have your vet tell you what to watch for though so you'll know if you need to get her in for an emergency c-section...have your vet's number and make sure he's on call for you.
Have them both fixed. Complications are possible but not very common. I'm sure they'll both be fine. Just make sure your vet is familiar with spaying tiny dogs
