I'm not sure I can offer any good advise with her relieving herself in the crate. I was thinking she was a young puppy and not able to hold it but at 12 months she should be able to hold it for a long time. Have you had her checked by a vet to make sure she doesn't have a UTI which would cause her to have to go frequently? Normally dogs won't go in their beds but I have seen others post with the same problems. The whole key to crate training is the fact that they won't relieve themselves in the crate. Other than having her checked by a vet and making sure her space in the crate is small enough that she can only stand up and turn around in it I'm not sure what other advise to offer.
