Here is Ellie's new recipe:
1 oz. white meat chicken
1/3 cup (or about 88 grams) carb (rice or sweet potato or peas...)
1/2 cup (or about 60 grams) veggies (squash, green beans, carrots...)
1 tsp. oil (for fat). I use canola.

You just cook it all and mix it. It is about 24% protein on a dry matter basis. You could definately go a bit higher in protein then that but Ellie has some health issues to consider...
I will either use a whole chicken and take off the white meat or boneless, skinless chicken breast. All skin and visable fat is removed.
The supplement I use is a bit harder to get than others. You have to order through your vet or a nutritionist, so it is probably best to look for other kinds. Calcium supplementation in particular is very important.
Some foods you never want to give are:
Chocolate
Onions
Garlic (in large amounts)
Grapes
Raisins
Macedamia Nuts
Foods high in fat or salt
