My Max is about 5 lbs and I've bought both Maxamillion clothes and Simply Dog clothes (a lot of them). The Simply Dog clothes fit perfectly and he wears a small in just about everything they make. I've never had anything run too big or too small in that line.
In the Maxamillion, I've gotten him mostly XSmall b/c they run quite a bit bigger. Not only that, there is a HUGE difference between the XSmall and the Small. I got Max the army vest top in XS and it's still pretty roomy particularly in the chest area (and he's got about a 14" chest). The only reason I didn't take it back was b/c the XXSmall is way too tiny for him. Hope this helps.
