Default Crate training problems - goes potty in crate

Max is 11 1/2 weeks old. We've been crate training since I got him at 8 weeks old. He's gone pee-pee in it several times and poo once. He hasn't gone in it as much as I'm sure he would because I take him out a couple times every hour while he's in it. (defeats the purpose, I know) Two times I've left him in his crate when we've gone out for less than two hours and both times he's gone pee-pee in it, despite having been taken out to potty right before. At night I don't lock him in it because I know he'd go like crazy in it. I leave it open in a play pen with wee-wee pads.

The crate has boxes in it so it's really only big enough for him to lay down and barely turn around. I can't figure out if it's an issue with him being able to hold it, or if he just doesn't care about laying in his own mess. At night he always comes out of his crate to go potty on the pads.

I know the point of crate training is teaching them to hold it and to go outside. But what do you do if they won't/can't hold it and go in the crate? Just keeping plugging away and hope he starts holding it longer? He seems to understand that outside is where he is supposed to potty. He doesn't tell me he has to go and if left to it he'd go inside but I really feel like he knows where he's SUPPOSED to go.

Anyone with suggestions?
