Do they come in pre measured invidal doses are do they get a vial and measusre into a syringe?

My friends son (16) went to get his and the lady was like oh this was empty, didn't you see me fill it? He was like no.

I thought they came pre done so to speak and think they used a used syringe. i tried to call the clinic to ask and the person I got to help me didn't know and was unable to find someone to answer my question. We called a different clinic who indicated they were indvidual pre measured dosages. Can different places use different options?
