When you are letting your puppy outside, do you know for certain that he is doing his business out there? In other words, do you make sure he has completely gone to the bathroom, before you leave him to his own devices? It maybe that he is going out there to play and nothing more. So my recommendation is to be in attendence during his bathroom breaks (your puppy does not know that these "outside" opportunities are bathroom breaks; he cannot reason that this is the only chance he will get before being crated for a number of hours).
Also, while yorkies need very little by way of excerise, they do need at least 15 minutes a day of good excerise, either with walking, running, or hard play. Simple day to day activity is not enough.
I know that Millie Sage needs to "walk it off" to have a bowel movement. She requires a little time and quite a bit of room to do it. So unless you are watching your little one, he may not be doing it outside at all.
