I would try what Rex's mom suggested. Then you could also slowly start having him hold it for short periods like if he wants out at 4:00 each morning wait until 4:15-4:30. Then if/when she starts waiting that long on his own bump it to 4:45-5:00. They eventually adapt to your schedule, it just may take some time. Reagan knows every morning she gets up at 5 and my husband lets her out. Then she lays back down with me in the bed and gets up with me a couple hours later. She also knows about when we goto bed at night and around that time she starts calming down.
If he has to go I wouldn't make him hold it for 4 hours, he may not be able to hold it that long. Start out a little at a time.
