It sounds like colitis, fluid builds up in the stomach and will eventually press on his lungs. Protein level is probably very high and my experience with Science Diet I/D should only be on a short term basis because if you read the ingredients, the nutrition is not good. Strongly advise another vet and there are links on YT about this condition. Leaking is definitely not normal, puncture is only a band aid for the core issue of the illness.
