Mot to sound rude but have you read any books on puppy training? I will be getting my puppy in December so I have been reading a lot of books on puppy training and "Puppies for Dummy's" has great information on house training, Also "The Dog Bible" has great information too. They explain how to use a crate for house training too.
Also I think you said that you are using vinegar to clean up the accidents, do not use it, it smells a lot like pee thus not getting rid of the smell and leading the puppy to belive that it's ok to pee their. OxiClean solution or Natures miracle works great as a cleaning product.
Also do not scold the puppy when it has accidents in the house, even a negative attention as scolding is, it is still a form of attention and the dog will then in a way be trained to use the house because it got attention in some form.
