You're very good to rescue the puppy. She may just be nervous, and not know what to expect, so you'll definitely need to prove to her that you will not hurt her. As for food, dogs don't generally starve themselves, so she will eat unless there's something wrong with her (in which case you see the vet). Just give it time, and keep leaving fresh food and water. Most likely because food has been changed so many times, she would rather just wait for something better than eat what's given. But that doesn't mean you should change the food. In the meantime, I agree...a little bit of Nutri-Cal should help.
With regard to the crate, a lot of the puppy mill dogs don't know that you can't go in the crate, because they're left there all day...they have no choice and that's what they think should be done. Also, keep in mind that "outside" is a very scary, weird place. She's probably never seen sunlight. I would suggest taking her out for a long walk, letting her explore different smells and sounds. Then, pet her and let her know that "outside" is good. They say that when you get a dog first, the best thing to do is take them out for a long walk before you get them home so that they don't think they are just going from one kennel to another.
