I've seen it before and it was really cute but I was concerned with infection or the pup taking it out. The lady said she had it done while the puppy was 10 weeks old and trained her not to mess with it. She said at that age it doesn't hurt them.
I was trying to see how she got it to stay in but couldn't tell.
