Here is what I did with Lily when I got her. She too was tiny and only weighed 8 oz and was the size of my cell phone. I couldn't get her to eat at all and was desperate to find something that she would eat.

I used the following recipe:
1/2 cup goat's milk
1/2 cup Gerber rice cereal
1 jar of Gerber's baby meat (I used beef)
1 tsp. karo syrup

I slowly added ground up kibble increasing the ratio of kibble to the above mixture every day. She seemed to like the Chicken soup for a puppy lovers soul the best. It was very small and flat so that she could eventually eat it by itself. Later she seemed to get tired of it so I started giving her Merrick's puppy plate.
