The open font could be anywhere on his head, even behind the ears. It sounds like he did do a neurological exam from your description. My main concerns right now would be hydrocephalus (either from an open font or head trauma which you may not have been made aware of), Liver Shunt, or MVD. The bile acid test will tell you if liver shunt or MVD is a possibility.
