if you are concerned, i would call the vet. as far as the lower nipples being full, place the babies there to nurse. as far as the water goes, i would keep putting the syringe in her mouth. mine had loose stools for about a week after whelping due to she ate all of the placentas.
keep trying with the food. give her a little at a time. i would put a little bit of bacon or hamberger grease on the food and mine would gooble it up. some people would not reccomend this but it worked for me.
i hope everything goes better for you. whelping is always scary.
