Default Behavior problem with Jack

I know puppies are supposed to chew a lot because they're teething, but Jack just will not give my fingers and toes or anyone else's a break. Whenever someone new comes to the house he immediately runs to them and starts chewing on their feet, if they pick him up, he chews on their fingers. He also tries to chew on my fingers and toes all day long. He has plenty of toys and bones, and I walk him for about 10 minutes 3 times a day and play with him constantly, so it's not like he's bored and doesn't get enough exercise or attention. I've tried telling him NO sternly, I've tried spraying bitter apple on my fingers and toes, but he doesn't care. I've tried holding him on his back. Whenever we play fetch, somehow he has gotten the idea that the object of the game is to bring the toy back, drop it in front of me and attack my hand viciously. I know that he is just playing, but it hurts! I have tried giving him toys when he chews fingers and toes, and he will play iwth them for about 10 seconds, then return to fingers and toes. Does anyone have any advice for how I can get Jack to stop this?
