I have the same problem with Lily who is very small due to MVD, hydrocephalus, and dwarfism. Her problem is the grass. If the grass gets higher than a couple of inches, then it is too high for her tastes and she refuses to go potty outside. She seems to have some kind of aversion to the grass, or anything for that matter, touching her belly. I have to have a special area in the yard that I keep mowed very short just for her. I leave a pee pad by the door for her to use when the grass is not to her liking.
As far as going to the bathroom in the crate goes, it is probably too big to be effective for housebreaking purposes. It should be just big enough for her to turn around in.
