Default house training

My Yorkie is one year old. He rarely has accidents in the house anymore, and when he is in his "room" (laundry room) while I'm at work he will hold it for hours and hours. My problem is that during the days I'm at home, he doesn't know how to "tell" me that he needs to go out. At night or in the morning he will scratch at the side of the bed or yip at me to let me know, but when he's just wandering around the house I have to keep an eye on how long it's been. I'm afraid to just "test" him and see how long it takes him to let me know, when I've tried that he has an accident. We hung a bell by the front door, and I ring it with his paw when we go out, but he won't do it on his own. Any advice???
