Maybe you're letting him have a little too much freedom at night. How long did you try the crate? While it's hard, try to be consistent with one thing instead of jumping from one idea to another. That's really confusing to Errol.
I have tried several different ways of housebreaking, but the thing that I try to do is to make it as impossible as possible to fail. That way, I'm never frustrated (which upsets and confuses the puppy and causes the whole problem to get worse) and my doggies always feel successful. It's hard work, but it you can make yourself stick with it, you will have more than 10 years of wonderful times with your baby without ever worrying about accidents!!
