Remember It's sometime best NOT to put your address on the tag.
People sometimes pet your dog or talk to you just to get a look at the tag and then can go to your home to steal your pet. There was another thread About this awhile back.
I don't think I put address on mine anymore just phone number.
Better to be safe.
