This is actually a very serious condition called paraphimosis - it is the inability to retract the penis into it's sheath. If you are not able to get the penis to retract, then you need to get him into the vet right away. If it is out to long, then tissue damage will occur and amputation may be required and most dogs do not fare very well after this.

I have seen some dogs where the owners thought that it would go back in on its own and amputation was required.

Try to get the penis to retract by using ointment to lubricate it or try the sugar. But, if it doesn't work, then get them to a vet asap.
