We have a stroller for Miko and Emma. They go on their regular walks, so they get their exercise, but if we go somewhere that they are not allowed to be on the ground on a leash like the mall, then we put them in the stroller. It would be too hard for me to try to carry an 11lb yorkie and a 4lb maltese in a carrier on my shoulders. Also, as mentioned before, it's a good place for them to lay down if they get tired at a park or at a friend's house...
