Peanut had this happen recently and I gave her a nice luke warm bath letting her soak in it for 15 minutes. After she was dried and relaxed I expressed some out of her swollen teat. Then put her in the whelping box and had the puppies nurse on that teat one at a time so they could help express the back-up of milk. In 1 hour I checked her teat again and placed a warm washcloth on it for about 10 minutes, then expressed some more milk again manually. Put her back in the whelping box and let the puppies nurse again. By morning it was totally back to normal and she hasn't had a problem since.
