Have all of his puppy teeth fallen out? If he still has any of his baby teeth, that will make their breath smell aweful! Even with brushing, food gets trapped in between the baby and adult teeth and plaque, tarter, and bacteria build up so fast. Lacy had to have her 4 baby canines removed around 7 months because they didn't come out on their own and before that her breath was so stinky.
