I don't have a yorkie yet but I have experience with other pups and cats and usually the stiches will get itchy once the wound starts healing, the pup will want to either lick or scratch at it to relieve him/her self.
My vet suggested a cone (the plastic collar) to put around the animals neck to prevent this. Most vet offices rent these out for a small deposit, but once the item is returned your money is returned. Be cautioned though most animals hate these and will fit it once it's on!!
