I had that problem with Mugzy, and when I asked our vet and she said, "1. Either they will or they won't come up, and 2. to help encourage them to come up, try shaving the hair off because it might be weighing them down while they are still little (Mugzy was only 3 pounds at the time)".
Also for treats, I only use a treat the size of about 1/2 of an M & M. Also, our trainer suggests "psyching" him out, start every other time or so, holding out your hand like you have a treat, quickly moving past her mouth and going into lots of petting with lots of verbal praise.
