Maybe soak his butt in a little warm water for about 10 minutes? Or at the very least, wipe and put a warm rag there. If his butt is hurting, that may help. (When Ozzie was having anal gland probs and his butt hurt, that's what we did and it seemed to help.)

I hate to think you'll have to take him back to the vet after everything he has already been through. Is he scheduled for any kind of a follow-up? If he continues to do poorly, I would get him back to the vet. That poor baby.

And poor YOU. I know what a toll these illnesses take on them and US. And not to mention the zoo you have taken on because you're such a loving soul. Lots and lots of prayers and positive thoughts headed your way.
I think it was more on the inside of his butt, cause his butt looked normal, no puffiness or redness. I know last year before all this stuff went on when his inside butt would get sore he acted funny and did the same thing running and hiding and kept looking back at his butt. so hopefully thats all it was. Hes been through so much , I know im going to freak every time he acts different.
