I have never had this problem but, this is what I have done for my little Lillie..

I bought a cat bed, the kind with a hood. I put the bed in the crate and pulled it all the way to the front of the crate. She really doesn't have any room in it but to sleep. She can stand up in it but, she doesn't have alot of room to roam. I made a little pillow for it and a blanket (for when it's cold). I make sure she does her nightly pee/poop, before she goes to bed. She has NEVER had an accident in her crate...This has worked really well for me.

Good Luck...I know these little buggers can be difficult sometimes!
