Warning: This might be graphic, I worked as a vet tech in highschool.
This only applies to male dogs. They make a small incision on the belly between the penis & the balls. then they press on the ball sack & the testicals pop out of the opening. Once visible they are tied off & amputated. The remaining part is pushed back into the hole & the hole is sued up. The whole precedure takes maybe 5 minutes.
Since all dogs are different swelling & the amount of extra skin would account for the difference in appearance.
