For a girl fur baby, clip or sew a very small bell to a hair bow.

For a boy or a girl, sew the bell onto a belly band and then put the band on baby, (place it closer to the head part rather than the tail part)!

If baby will leave it on, you could make a garter to put on baby's leg, and sew the bell to that.

I Have not tried any of these, just brain storming!
