Thanks for the tips. That makes me feel better. I do normally reinforce the inside of the fabric (in between the "real" fabric and the lining) and inside the d-ring strap with medium-weight interfacing. Maybe I'll double up on that or get a heavier interfacing. I normally use about 3" of velcro, but I think I'll try the 4" for this one.

Omega: I've had Chloe pull right out of another velcro harness, but it had only about 1 1/2" square of velcro. Was the hoodie a knit fabric? I stay away from knits...I don't really like sewing with them much anyway.
