I would start back at square one and restrict his freedom. It happens. My pup (almost 6mos) hasn't had an accident in the house for over 3wks (he is being trained to outside). Last night in a split second I saw a puddle on my floor. I didn't see him do it, so I just cleaned it up and took him outside. Inside, there is always that little feeling of defeat (and frustration) but, then you just say..he isn't ready yet or reliable. Because I am a mom, I always feel better when I relate it to human toddlers (who actually understand the human language)..Even toddlers, you think they are ready for underpants, they do good for a week and then bam...they poo and pee in their undies and you start all over....
