My shih tzu Georgia was probably around 4lbs. when she got spayed and did fine. The only problem was she was so darn active for the next few days that the surgical glue holding together her incision started to come unglued! Her incision just looked like it was splitting open a little so off we went and they put some more glue on the incision. The biggest thing is keeping them quiet for the next week or so that it takes for them to heal!! They feel so good and normal within a short period of time and don't know why we're keeping them down. Here's the answer to your questions from my experience
1. No, my vet did not recommend antibiotics as a precaution
2. My vet does not routinely give out cones unless licking of the incision becomes a problem. Neither one of my dogs was sent home with an e cone and did just fine.
3.Yes, she was on pain killers for the rest of the day and two days after.
