I am so sorry to hear your little guy is so sick. If he is in a lot of pain and surgery would be dangerous (maybe because of his cough?), you may consider euthanasia. But I would give it a few days before making that decision, to see if maybe the knee pops back in and the medicine takes effect in healing his cough. You may also want to seek a second opinion from another vet.
