On a different note, I was also told to NOT clean up the dirtied crate in the presence of the dog because he would just think "oh, it will get cleaned after I dirty it" and it will be thought of as a way of getting special attention. I was told to put him in a different room and clean it.
Here's my current schedule:

1) 5:30AM Out right when we wake up (taken out of crate), on leash
2) 5:45AM Feed time
3) 6:00AM Out right after eating, on leash
4) 7:00AM Out right before I leave for work, on leash
5) 12PM Out during my lunch hour (taken out of crate), on leash
6) 5:45PM Out right when I get home (taken out of crate), on leash
7) 6:00PM Feed time
8) 6:15PM Out right after he eats, on leash
9) Various evening trips outside, for wandering, off leash
10) 12AM (night owl), out right before going to bed (crated), on leash then set free to wander after he goes.
REPEAT
