The Lactulose helps, but with MVD and Liver Shunt, some part of the liver is still functioning on it's own. The liver can also rejuvenate, so there may come a time when the dog can handle a lower protein/ hepatic diet on it's own. All depends on the extent of any permanent liver damage. Also an FYI: A dog can live and do well with just 1/4 of it's liver functioning normally.
