Sounds like anxiety/restlessness. I'd see what the neuter does. If after that you have tried all you know to deal with the anxiety or whatever it is, then drugs are always an option.
One of my dogs is a compulsive chewer...not just a puppy thing. He was hurting himself and making things very difficult for us. So he is now on a low dose of Prozac and is doing so much better. Worth a try...
