I had this issue with Holly for the longest time. Most often it occurred after we got out of bed in the morning and she got excited and got on the couch and peed before she went to her pee pad. I restarted the training and physically carried her to her pad and made her stay until she went before she got to run around and get on the couch. I also had a brief time where she would pee on the couch during the day, again I think it was out of excitement and forgetting to go to her pee pad. I banned her from the couch entirely until she could prove to me that she can use her pad at all times. If I was not at home and not able to watch her, she was kept in her ex-pen or the sofa was covered with plastic or filled with boxes or something so she could not get up there. Knock on wood she has not peed the couch now for several months. You need to be one step ahead, figure out the cause and go after it. In between, make the couch an unpleasant place to be. Being on the furniture is a privilege that must be earned. Good luck.
