its just safer. If you cannot watch the dog and tehy really wanted to go at it they could bite thru the onesie. The onesie is just a thin layer of cotton which puppy teeth can go thru easily. Biting thru the onesie but not ripping it can still cause irritation to the surgery site. with the cones on there is no way they can get at it.

clothing is easy to get thru. i know of a golden retriever who had a mass removed on his chest area. The owners put a shirt on him to prevent him from getting at it. well he ate thru the shirt and then ate a hole in his skin. don't want that happening.
