| most likely it is the 'puppy application form you have to fill out to send in to get her actual registration certificate. this will have her date of birth and the names and registration numbers of both parents. the breeder will sign it on the back that she has transferred the puppy directly to you and you will fill out your name, address and new puppy's name. then you pay a very small fee and send it in and in about two or three weeks, you get your registration certificate back. |

| The only papers I know of are the Registration papers. The breeder completes part with the names, registration numbers and other info of the parents. Then there are documents that you fill out to register your puppy with the AKC or other registration agency. You might do a search here on Registration and that will give you lots of info. Many breeders won't send you the papers until you send them proof of spay/neuter. That should be spelled out in the adoption/breeder contract.

| Are you talking about the registration papers, or the papers that the breeder gives you with the puppy?

| You should get registration papers, a health guarantee, and a copy of the shot records.
