| Puppy applications, Questionnaire form. I am trying to design a Puppy Application Form. I don't want to forget any important point so I was wondering if you breeders could show me the form that you are using. It would be very helpfully to me and extremely appreciate. |

| You are very welcome! Feel free to use it and tweak it to what your comfortable asking, and add whatever I might have left out. I just use that for step one. Caretaker and Family Information: Name and Ages of Other Family Members: Are ALL members of the family ready for a new dog? If you have children, are they properly educated on how to handle small dogs? What are the ages of children in your home? Do you allow your children to play with your dogs unsupervised? Who will be the primary caretaker for the new puppy? What is your working situation: Do you and/or your spouse work full-time? How much time each day do you expect your new Yorkie to be at home alone: Do you and your family have an active/outdoorsy or relaxed/leisurely lifestyle? Would you describe your family's household as active/busy or quiet/relaxed? Residential Situation: Do you live in an apartment or house? Do you have a garden? What is the size of your garden? If you are renting, does your current lease allow you to have pets? Where will the new puppy sleep? Where do you plan to keep the puppy when you are unable to supervise? Are you planning to use an expen (highly recommended) or a crate to provide the puppy with a place to stay? Current and Future Pet Situation: Are there other Animals in your Home?
