A 12 week old puppy is just like a toddler. You certainly wouldn't be upset with a human child if they had an accident. In fact, most parents make a really big deal out of going in the potty. We clap and say "yay" and make sure we tell daddy when he gets home what a great job baby did going in the potty and we don't mention the mistakes at all. When your toddler has to go potty, they have to go NOW and if they don't make it, it should be no big deal, right? And that's exactly what your puppy should be experiencing. When she has to go, she needs to go NOW and it's your job to watch for the signals. If she has an accident, guess who's fault it is? Not hers. She only wants to please you so lots of very yummy treats for appropriate potting. I would make up something really special like turkey bacon or liver, none of those store bought things but figure out which is her favorite and reserve that for going potty in the right place. And when she goes in the right place, not only does she get lots of great treats but a party with clapping and petting and telling her what a great job she did! For now, just ignore mistakes... And also continue way past the point where you think she gets it because at first it will be to get a yummy treat but eventually it will become a habit so make sure you follow this routine until it does in fact become a habit.
