I am so sorry you are having this problem, but she has somehow forgotten?? Maybe confine her in a smaller area. It may take a good while again. Bella did this too when she was trained because I gave her a larger area of the house. So I closed off a smaller section. It worked for her. Enforce her good behavior every time. Reward her when she goes on puppy pad. We make a big deal everytime she goes to reinforce and remind her. Clapping hands, "good girl", and "wanna treat". They really react to attention.
