I've seen many xxx-smalls at Walmarts...much more frequently than than the small size I need anyway. Collar wise...you may want to look at a break away collar for cats. They're much smaller than dogs and if it caught on something...it comes apart.
Another clothes suggestion...cut the heel and toe off a sock, clip two arm holes...instant sweater.
